Replacing a passport when changing a surname

When changing the surname, name or patronymic, the passport automatically becomes invalid and must be replaced, i.e. replacement of a passport is a mandatory procedure when changing a surname, name or patronymic. Passport with access form and secrecy

Rules for filling out the questionnaire

  1. Surname First name Patronymic - must be written as in the passport, without declension:

  2. Gender - put a mark in the appropriate box.
  3. Date of birth - write the day, month and year in the format DD.MM.YYYY:
  4. Place of birth - as in the passport:

  5. Information about changes in the name, surname, patronymic or field - put a mark in the appropriate column, then, if changed, indicate what and when.

    If you have changed your full name more than once, indicate the last change. All previous changes are indicated in Appendix 2 to the questionnaire ().

    Don't forget to check the "No" box if you haven't changed anything.

  6. Address of residence - information from the registration stamp in your passport is indicated.
  7. Address - enter information about the actual place of residence.

    If you have a temporary registration, put a checkmark in front of the field "place of stay", if not - "actual residence".

    Fill in all the points only if you are applying for a passport not at the place of registration. If you live where you are registered, you do not need to fill out anything.

  8. Phone - a number where you can be contacted.
  9. Email address - fill in as you wish.
  10. The main document proving the identity of a citizen of the Russian Federation (passport) - fill in all the items in accordance with the existing passport:

  11. Clearance to Confidentiality - Specify "No" if you have never been cleared to sensitive or top secret information.

    If you had such a permit, check "Yes" and indicate its form, as well as the organization that issued it and the date of issue.

  12. Note if you have contractual or contractual obligations that prevent you from traveling abroad. Check "Yes" or "No". If yes, indicate the organization and year of signing the contract.
  13. Information about previously issued passports - put a mark in the appropriate field.

    Fill in the fields with the details of your most recent passport. If you have lost your passport, it is still advisable to indicate its data. You can rewrite this information from the seal in the civil passport, which is placed on pages 18-19 in information about previously issued documents.

  14. Enter information about your work activity for the last 10 years. Write not only information about work, but also about places of study and military service.

    Please be honest with all information on the application form. Although when submitting documents now a certificate from the place of study and a work book are not required, however, incorrectly indicated information may cause additional questions and a refusal to issue a passport.

    If you had breaks in study / work for more than one month, be sure to indicate “TEMPORARILY NOT WORKING” and write the address of registration at that time.

    If you had a transfer to a new position, you should indicate the last one. If your organization has changed its name, please indicate the last name.

    The address must be written without a zip code. Recording format - region, city, street, house. You can specify the actual or legal address.

  15. If all the information does not fit in the table, write it down in Appendix 2a to the application for a passport ().

We print the questionnaire on one sheet (applications, if any, are printed separately), using two-sided printing. Then we put a signature. It should not go beyond the boundaries of the specified rectangle. Your application form will not be accepted if the signature goes beyond the indicated limits.

You do not need to stick a photo, and also put the date of completion at the bottom of the questionnaire. All this for you will be done by the staff of the FMS.

New generation passport for a child

There are separate rules and requirements for obtaining a foreign passport for minor citizens of the Russian Federation. In addition to the main package of documents that is necessary for obtaining a passport, you will need a birth certificate of the child to confirm the rights of the legal representative and, in the case of adopted children, an act on the appointment of a guardian or trustee from the guardianship and guardianship authorities.

The passport application form for a child is slightly different from the application form for an adult citizen of Russia. For example, there are no items on education and work in it. On the reverse side of the questionnaire, the personal data of the legal representative of the child are filled in.

In general, the filling rules remain the same as when filling out an application for a passport for an adult.

When applying for an old or new international passport, the applicant will be required to submit to the relevant authority (MFC, passport office, Russian embassy) a set of documents: a copy of the Russian passport, photographs 35X45 on matte paper, a copy of a valid international passport, paid state duty and a completed passport application form. Why are application forms needed?

When filling out the questionnaire, it happens that in the application in paragraph No. 5 or No. 14 there are not enough lines to indicate all the information. For example, there are many official jobs or a frequent change of surname. If the applicant had to repeatedly change his surname, then without fail you will have to use Appendix No. 2. If there is not enough space to provide information about the applicant's work activity over the past ten years, then Appendix No. 2a comes to the rescue.

Appendix No. 2 is used only for information about the change in the applicant's personal data: last name, first name or patronymic. Appendix No. 2a is used only for information related to the applicant's activities over the past 10 years: work, study, serving a sentence, service, etc. This is where the differences end.

Validity of the questionnaire

Since the questionnaire provides data on labor activity for the last ten years in the form “month.year”, the validity period of the completed questionnaire is about a month, depending on when you filled it out. Let's say you fill out the questionnaire in December 2018, so the data will be indicated from 12.2008, where 12 is the month and 2008 is the year. The questionnaire completed on 12.2008 must be submitted before January 2019.
