There are two forms for adults: an application for the issuance of a new generation passport - filled out when making a new type of foreign document and an application for issuing a passport - is used when issuing an old-style international passport. A new generation passport is issued for a period of 10 years, an old one for 5 years. A foreign passport of a new type (electronic, biometric) differs from the old one in a greater degree of protection. The applicant draws up personally in one copy. The procedure for filling out an application for issuing an old-style passport
The applicant draws up personally in two copies. Filling out is similar to the application form for issuing a new generation passport. The only difference is the presence in the application for an old-style international passport of paragraph 15 - “Information about children under the age of 14”, data on which are additionally entered in Appendix No. 2b.
When applying for an old or new international passport, the applicant will be required to submit to the relevant authority (MFC, passport office, Russian embassy) a set of documents: a copy of the Russian passport, photographs 35X45 on matte paper, a copy of a valid international passport, paid state duty and a completed passport application form. The questionnaire contains information about the applicant: full name, address of registration, information about labor activity. After your documents are accepted, the questionnaire is sent for checks through the lines of the Ministry of Internal Affairs and the FSB. Validity of the questionnaire
Since the questionnaire provides data on labor activity for the last ten years in the form “month.year”, the validity period of the completed questionnaire is about a month, depending on when you filled it out. Let's say you fill out the questionnaire in December 2018, so the data will be indicated from 12.2008, where 12 is the month and 2008 is the year. The questionnaire completed on 12.2008 must be submitted before January 2019.

Filling by hand: which pen and how?
You can fill out the questionnaire by hand, but only in blue or black ink, in capital block letters, without corrections, strikethroughs and proofreaders.
It is more difficult to fill out by hand, the handwriting should be extremely clear, and if you make a mistake in at least one place or one letter, you will have to rewrite everything again, so it will be easier for you to download the questionnaire and fill it out on a computer.
In some departments of the Federal Migration Service, your application may not be accepted at all., filled out by hand, if they think that the equipment will not be able to parse your text (especially if you have incomprehensible handwriting).
